A decision is opened
It starts with a question, not a task.
A decision record opens with the question being decided, what is at stake, and the date it has to be answered by. Opening one is a deliberate act: somebody has said, on the record, that this is a call the company is making.
Nothing has been decided yet. That matters: the record exists before anybody knows the answer, which is the only moment at which an honest account of what was believed can be captured.

When one of the facts turns out to be false
The one part of the story the real data has no example of, so it is described rather than shown.
A fact can be marked false long after decisions have relied on it. Clyreon never deletes or rewrites the original claim, and it never declares the decisions that used it wrong. It answers a narrower and more useful question: which decisions are now worth another look, and who owns them.
That includes decisions in other teams that never used the fact at all, and only relied on a decision that did. This is the clearest reason the memory is shared rather than split up by team: a mistaken assumption in one function reaches commitments made in another, and a memory kept in nine separate places cannot follow it there.
The claim was wrong. The original is never deleted or rewritten, only marked, so the record of what was once believed true stays intact. This is what puts decisions in doubt.
A more accurate measurement of the same thing arrived. Earlier judgments were working from the best available at the time, so this puts nothing in doubt.
Somebody disagrees with the claim. The disagreement is recorded beside it, not in place of it. Doubt written down is not a verdict.
A decision that used the false fact. What is known is narrow and stated as such: one thing it rested on was wrong, so the decision is worth another look.
A decision in another team that never used the fact at all, and only relied on a decision that did. This is how one team's mistaken assumption reaches a commitment made somewhere else.
Being affected does not mean a decision was wrong. Whether it still holds is a question only a person with the authority to judge can answer, by recording a new judgment, which stands beside the old one rather than replacing it.

Opened, argued over, sealed, answered by reality, and still readable long after everyone involved has moved on.
No health score, no recommendation engine, no assistant deciding anything. What makes this different is not a feature list. It is that none of it can be edited afterwards, and that the outcome attaches to the original prediction rather than to a version written once the answer was known.
